Why Reasonable Security Decisions Still Lead to Incidents
The most dangerous security failures don’t start with negligence. They start with confidence.
Most security incidents don’t start with bad decisions.
That’s the uncomfortable truth no one likes to hear.
They start with reasonable ones.
Decisions that looked correct, responsible, and aligned with best practices.
Decisions that were reviewed, approved, and documented.
Decisions no one felt the need to challenge - until something broke.
That’s what makes them dangerous.
Not ignorance.
Not negligence.
But confidence.
“On paper, this was the right call.”
I’ve seen this pattern repeatedly.
A system is designed with security in mind.
Controls are selected based on risk assessments.
Budgets are respected.
Timelines are realistic.
Nothing reckless.
Nothing obviously wrong.
And yet, months or years later, there’s an incident.
Which is usually when the phrase
“no one could have predicted this”
quietly enters the room.
Not because someone ignored security -
but because everyone assumed the decision was good enough.
Why the decision made sense
Most security decisions are made under constraints:
limited budgets
pressure to deliver
incomplete information
competing priorities
organizational politics
In that context, choosing a good solution instead of a perfect one is not a failure.
It’s normal.
In fact, it’s often the only viable option.
This is why looking back and saying “we should have known” is misleading.
At the time, the decision was defensible.
Hindsight doesn’t make decisions worse.
It just makes responsibility clearer.
That’s the uncomfortable part.
Incidents rarely come from one mistake
When incidents happen, people look for the cause.
A misconfiguration.
A missed patch.
A user clicking the wrong link.
But in reality, incidents emerge from alignment failures:
assumptions that were never written down
responsibilities that were implied, not assigned
risks that were accepted quietly
controls that worked, just not together
Nothing failed catastrophically.
Everything failed slightly.
And no single team felt responsible enough
to stop the system from moving forward.
Those small gaps lined up.
The real issue isn’t technical
Most post-incident reviews focus on tools and controls.
What’s often missed is that the real problem sits higher:
Who actually owned the risk?
Who decided the risk was acceptable?
Who would have been accountable if things went wrong?
Many security decisions optimize for delivery and compliance,
but not for failure handling.
They answer:
“Is this good enough to move forward?”
But not:
“What happens when this fails, and who carries the consequences?”
A simple way to pressure-test “reasonable” decisions
Over time, I’ve learned that reasonable decisions fail
not because they’re wrong,
but because no one forces them through a second lens.
Before accepting a security decision as “good enough”,
I now mentally check just three things:
What assumption would hurt us the most if it turned out to be wrong?
Not the technical one, but the organizational one.Who explicitly owns that assumption?
If no name comes to mind, that’s already a risk signal.What would we say after the incident to justify this decision?
If the explanation sounds defensive or vague,
the decision probably needs another round of thinking.
This doesn’t slow projects down.
It slows down false confidence.
Experience changes how you think about security
This is where seniority in cybersecurity shows.
Not in deeper technical knowledge,
but in how decisions are framed.
Experienced professionals don’t ask:
“Is this secure?”
They ask:
What assumptions are we making?
What are we accepting?
Who pays if we’re wrong?
That shift doesn’t come from reading more standards.
It comes from seeing reasonable decisions fall apart.
Why this matters for CISSP (and real life)
This is also why CISSP feels uncomfortable for many technical people.
Not because it’s unfair,
but because it refuses to reward purely technical thinking.
The exam isn’t interested in what could work.
It cares about what happens when things don’t.
CISSP questions often reward answers that feel conservative, slow, or bureaucratic,
because those are the answers that survive:
incidents
audits
lawsuits
It’s not about being clever.
It’s about being accountable.
The quiet lesson
Most security failures don’t start with negligence.
They start with decisions that felt safe enough
to stop thinking about.
That’s why they’re so hard to detect early.
And why they repeat.
Security doesn’t fail loudly.
It fails quietly,
and then demands explanations later.

Thanks Erich. This is great advice. In academia the biggest issue we have is a complete abdication of responsibility. The security training we have to complete is just so that the institute can say we are 'compliment' when a breach occurs. None of it is actually contextual or teaches anything of merit at all. 😢
